چکیده

Public health systems aim to improve the health of a population at a cost whose limits are set, one way or another, by society. To ensure that the health of a population is improved, clinical procedures that are supported by scientific evidence of a more favorable outcome (risk/benefit ratio) compared to any other alternative should be used. To ensure that the outcomes are achieved at an acceptable social cost, the cost of obtaining an outcome with each of the technically possible alternatives should be known. The outcome of a health intervention can be measured by efficacy (in ideal experimental conditions), effectiveness (in real everyday conditions), utility (survival time adjusted for quality of life) and benefit (outcomes expressed in monetary units). The best scientific evidence on the outcome of therapeutic interventions comes from randomized clinical trials with no methodological weaknesses. That is, clinical trials provide information on the efficacy of the interventions. Efficacy is necessary for effectiveness but it alone is not sufficient. Effectiveness depends not just on efficacy but also on local factors, which may differ from those of the clinical trial (technology, experience, organization, etc). Efficiency is the ratio of the outcomes to costs that have to be met to achieve the outcome, thus classic efficiency analyses evaluate cost/efficacy, cost/effectiveness, cost/utility, and cost/benefit.1 When a new technology appears in medicine, the key questions focus on the outcomes and the costs with respect to existing technologies, though the answers to these questions do not usually come quickly. Thus, some effective techniques spread slowly ED I TO R I A L S

Safety and efficacy of coronary drug-eluting stents in octogenarians.

INTRODUCTION AND OBJECTIVES The effectiveness of coronary drug-eluting stents has not been fully studied in very elderly patients who have clinical features that predispose them to side effects and long-term adverse events. METHODS The safety and efficacy of drug-eluting stents were studied in a historical cohort of consecutive octogenarian patients who underwent percutaneous coronary interve...

Long-term effectiveness and safety of sirolimus drug-eluting stents

The root cause of coronary artery disease is atherosclerosis, ie, intraluminal narrowing (stenosis) of the arteries that supply blood to tissues of the heart. The introduction of the drug-eluting stent over the past decade has revolutionized the field of interventional cardiology. It is used extensively in clinical practice for the treatment of coronary artery disease. The first drug-eluting st...

Drug-eluting stents: results, promises and problems.

In-stent restenosis is the major drawback of percutaneous coronary interventions, occurring in 10-40% of the patients. Recently, new stents have emerged which are loaded with anti-inflammatory, anti-migratory, anti-proliferative or pro-healing drugs. These drugs are supposed to inhibit inflammation and neointimal growth and subsequently in-stent restenosis. In this review article the results of...

An advection-diffusion multi-layer porous model for stent drug delivery in coronary arteries

Arterial drug concentration distribution determines local toxicity. The safety issues dealt with Drug-Eluting Stents (DESs) reveal the needs for investigation about the effective factors contributing to fluctuations in arterial drug uptake. The current study focused on the importance of hypertension as an important and controversial risk factor among researchers on the efficacy of Heparin-Eluti...
